What is a FHO or Family Health Organization ?

A Family Health Organization is an initiative of the Ontario Medical Association and the Ontario Ministry of Health and Long Term Care.

How does an FHO work?

Your Family Health Organization (FHO) is a group of family physicians who work together giving your family best access to personalized care and quality services – with a focus on illness prevention.

FHOs provide care during regular and extended office hours and patients also have access to a nurse-staffed Telephone Health Advisory Service.

For FHOs to be effective, enrolled patients must commit to receiving their primary health care Monday to Friday from their primary physician or, if he/she is not available, from another doctor in the FHO.

Additionally, as a patient of an FHO, you will have access to referrals to specialists and having your doctor informed of your care both in and out of the hospital. Our doctors are also affiliated with Trillium Health Partners, so if you are ever admitted to hospital and whenever it is possible your family doctor may come visit you for continuity of care.

What Does This Mean For Patients?

When you are a patient at Dixie Road Medical Associates, you may access:

  •   – Ability to call for same day appointments ( FHO Appointments) based on availability.
  •   – Requests for us to advocate for you to get the tests and services you need.
  •   – Requests for us to make appropriate referrals on your behalf.
  •   – Telephone health advice 24/7 through the Telephone Health Advisory Service.
  •   – Extended hours on evenings and weekends
  •   – Visit from your family doctor whenever possible, if you’re ever admitted to Trillium Health Partners ( Mississauga Hospital ) for continuity of care.

Choose Your Doctor

Several of our physicians are currently accepting new patients (Dr. Karnik, Dr. Attalla, Dr. Nahirnyj and Dr. Ona-Olapo).  Please fill out a new patient registration form indicating your interest in becoming a patient at Dixie Road Medical Associates.

Complete the registration form to be placed on our waiting list

Please complete the registration form by clicking on the register here button.  You will be placed on our wait list and we will contact you to book you for a new patient intake appointment with one of our physicians.  Please note that the average wait time from registering to appointment time is between 2 to 8 weeks.

Step 3

Book your first appointment

Several of our physicians are currently accepting new patients (Dr. Karnik, Dr. Attalla, Dr. Nahirnyj and Dr. Ona-Olapo).  By completing a registration form you will either be called to schedule a new patient appointment, or be put on our waiting list to be contacted when it is your turn to be assigned a new patient appointment.

Keeping You Well

Thank you for trusting us with your health needs.

Please remember to always come 10 minutes early to your appointment, and bring a VALID health card and all of your medications to each visit.

If your health card is not valid, or if you are late for your appointment, you will be charged the appropriate OMA rate for your meet and refunded when your health card has been updated, or your appointment will be rebooked.

If you are age 50 to 74, the Ontario Breast Screening Program recommends that most women in your age group get screened with mammography every 2 years. If you are 30-69 and you are at high risk of breast cancer please discuss this with your doctor.
Women should begin screening for cervical cancer at age 21 if they are or have ever been sexually active. Women who are not sexually active by age 21 should delay cervical cancer screening until they are sexually active. Sexual activity includes intercourse, as well as digital or oral sexual activity involving the genital area with a partner of either sex. Screening is every 3 years until age 70 unless there is history of abnormal results.
