New Patients : Please complete and sign a new patient intake form. You can either download, fill in and email back the form below or the form on our “Join Waitlist” button on the clinic home page to newpatientregistration@drma.ca, or you can bring a form into our office to register as a new patient. We also ask that you do the Virtual Care & Email communication policy and consent form giving your permission to communicate over the phone and distribute materials by email. You will also be required to do a New Patient Declaration form, and we can assist you with that when you come to the office for your first appointment. The New Patient Declaration tells the Ministry of Health that you are now enrolled with your doctor at our clinic. Any forms you do before your first appointment can be emailed back to us here.
